All athletes must register here: www.athleticclearance.com to complete the new electronic athletic packet.
Reminder, you must choose the 2024-2025 school year. This option will open up either late May or early June 2024.
If your student-athlete has asthma, please download and print the Self Medication Form above. The forms require a medical doctor's approval and signature.
Please remember, double check for all signatures, dates, and doctor stamps.
Incomplete Athletic Packets will be returned.
