Athletic Packet - Forms
Please download the 2025-2026 Athletic Packet and complete the tasks and turn it into your coach.
Step 1: All athletes must register here: www.athleticclearance.com to complete the electronic athletic packet. Upon completion, you will receive a Confirmation Page. That confirmation will need to be printed and signed by the athlete and a parent/guardian.
Step 2: Provide a printed copy of an insurance card (front & back),
Step 3: Provide the (2) page sports physical (click below and print). Sports Physical Form must be an LAUSD issued form and page 2 must be completed by a physician.
Step 4: Provide steps 1, 2, 3 to the Coach to be processed.
Please remember, double check for signatures, dates, physician stamps.
Incomplete Athletic Packets will be returned.
If your student-athlete has asthma, please download and print the Self Medication Form below.

FREE Athletic Physicals
ATHLETIC PHYSICALS
Each athlete must have a current physical on file in order to play sports. Physicals may be obtained through your own physician but MUST be on the LAUSD form.
Physicals are good for 1-year (365 days). The physical (original only) will then need to be turned in to the coach of the sport you wish to tryout for, then it will be cleared by the Athletic Director and school nurse.
Make sure that the Physical is filled out completely; signed, dated and stamped by the Doctor's Office. If you have an Asthma condition or take any type of Asthma medication, please print, complete and turn in the Asthma forms below along with the Original Physical form.
